OK, so not being able to get an erection is bad enough but what would you think if your heart was limping along, possibly ready to give up the ghost at anytime?

Researchers from Boston University School of Medicine (BUSM), in collaboration with colleagues at Lahey Clinic and from Denmark and Germany, have found that 5a-reductase inhibitors (5a-RIs), while improving urinary symptoms in patients with benign prostatic hyperplasia (BPH) and possible hair loss prevention, produces significant adverse effects in some individuals including loss of libido, erectile dysfunction (ED), ejaculatory dysfunction and potential depresssion. These findings, which currently appear on-line in Journal of Sexual Medicine, suggest that extreme caution should be exercised prior to prescribing 5a-RIs therapy to patients for hair growth or for BPH symptoms.

5a-RIs, finasteride (Propecia™) and dutasteride, have been approved for treatment of lower urinary tract symptoms, due to BPH, with marked clinical efficacy. Finasteride is also approved for treatment of hair loss (androgenetic alopecia). Although the adverse side effects of these agents are thought to be minimal, the magnitude of adverse effects on sexual function, gynecomastia, depression, and quality of life remains ill-defined.

In order to determine the potential extent of this problem, the researchers examined the data reported in various clinical studies concerning the side effects of finasteride and dutasteride. After a review of the literature the researchers found that in a subset of men, prolonged adverse effects on sexual function such as erectile dysfunction and diminished libido were reported, raising the possibility of a causal relationship.

According to the researchers, the adverse side effects of 5a-RIs on sexual function, gynecomastia and the impact on the overall health have received minimal attention. However, in some patients, these side effects are persistent with regard to sexual function and with an emotional toll including decreased quality of life.

"The potential widespread use of 5a-RIs for treatment of BPH, prostate cancer and male pattern hair loss may produce undesirable adverse side effects on overall health and in particular, vascular health and sexual function in a subgroup of susceptible patients, " said lead author Abdulmaged M. Traish, MBA, PhD, a professor of biochemistry and professor of Urology at BUSM. "Furthermore, treatment of hair loss, a benign condition with 5a-RIs may produce persistent side effects in a number of young patients," he added.

"Honest and open discussion with patients to educate them on these serious issues must be pursued prior to commencing therapy because, in some patients, these adverse effects are persistent and may be prolonged and patients do not recover well after discontinuation from drug use," cautioned Traish.

The researchers believe additional clinical and preclinical studies are warranted to determine the reason for why some of these adverse effects persist in some individuals.

We appear to need around 90 to 100 substances daily, as a basis for good health and a long life! Intensive modern farming methods on barren soil deliver grossly polluted food, water and air.  Mix this little lot in a bowl, together with the abysmally poor regulation of food producers and retailers by a complacent government.  Add in too much stress, too little sleep, poor preventative health care, lack of exercise, and the over-use of antibiotics, electromagnetic pollution and the suppression of positive nutritional information by vested interests.  What comes out is a problem that is staring us in the face in ten-foot high letters, namely, disease & interventive healthcare costs are out of control, period!

The majority of food produced for our consumption is literally unfit for ingestion.  It is highly processed, cleverly packaged garbage.  It is full of sugars, hydrogenated fat, artificial flavourings, dyes, additives, fillers, binders, excipients, anti-caking agents, artificial sweeteners, bulking agents, solvents, bleaching agents, emulsifiers, gelling and glazing agents, stabilisers, preservatives, antibiotics and growth hormones! The list of non-food ingredients that by law is allowed in our food is almost endless.  If that isn't enough, when crops are growing they are sprayed with many differing fertilisers and insecticides, among other types of chemicals.  These become part and parcel of our food, while the residue enters the water table, so we get a double dose for good measure.

Fertilisers and insecticides can act as synthetic oestrogens, once they are in the body.  This can lead to severe damage to both males and females.  Would you believe that an emulsifier (nonyl phenol) used by many plastic container manufacturers, can leach into the food or liquid, contaminating both the food and the body of the unlucky person it enters?  Nonyl phenol that is leeched into foods  is also known to act as a synthetic oestrogen.  Triple dose.

Testosterone and Bodybuilding - The King of Male Hormones

Testosterone is the ‘king’ of male hormones – it make up our maleness in varying degrees.  Tess produces an anabolic (building up) environment as well as enhancing libido to, in some cases, an overwhelming degree.  This is not as good as it sounds! It also affects mood (‘roid’ rage), deepens the voice , increases bone density, reduces body fat etc.

In bodybuilding testosterone is the starting point for increased muscle mass as long as the correct dietary intake of foods such as protein are adhered to.  Taking tess alone without loading the muscles will not create gains.

Without all the relevant nutrients the body cannot manufacture sufficient tess. Some years ago testosterone supplementation was considered to be a no-no.  However today it is available on prescription if it is found to be required.  Such is the nutrient deficiency in a modern diet that younger & younger males have a raging tess deficiency, leading to all kinds of problems.  By utilising tess supplements, gels, creams, injectables etc, many problems can be overcome & that is good news.

Unfortunately many body builders think that if they double or quadruple the normal dose (particularly with injectables) then they will make gains that much faster

This is folly. Severe hormonal changes can result which affect many internal & external processes.  The most common that people are aware of are ‘roid’ rage, skin, conditions & severe emotional swings. The heart, liver, & kidneys may also be adversely affected. Perhaps not widely known out of body building circles is that males can become feminised inasmuch as they can develop ‘gyne’ or breast tissue.  This is due to an excess of oestrogen being converted in the body, through a process known as aromatisation, whereby testosterone is converted to oestrogen.  This excess tissue can only be removed by surgery.    This is bad news.

Hormonal production in the body works via a feedback – loop system.  In other words it shuts off production of hormones when a certain level is reached to keep within required parameters.  When testosterone is introduced into the body from external sources, the internal production of it will be down regulated.  With prolonged use the testicles will begin to shrink as they shut down because they do not need to manufacture testosterone.

Shrinking testicles may well not be a problem to some people.  However, it becomes a problem when the external supply of tess is stopped.  It can take up to two years for the testicles to produce normal amounts of it.  This is ugly.  The loss of libido can be profound & can have far-reaching affects.  I have seen this in my Naturopathic practice on numerous occasions.

A word here on female body builders using testosterone.  It has the opposite affect to that seen in men, in that it tends to masculise them.   The voice deepens & hair growth accelerates everywhere.  Generally the libido goes off the chart & the clitoris increases in size, dramatically.  This is good, bad, or ugly depending on your point of view.  Obviously, excess tess can also produce the same negative health affects as those experienced by men.

If you are going to use testosterone for body building it is far better to cycle it – 2 weeks on two weeks off and stick to the recommended dose.  Sure, it will take longer to get the gains you seek, but you will get them & preserve your health & longevity.

Male Sweat (pheromones) Boosts Women’s Hormone Levels

In two trials, a total of 48 undergraduate women at UC Berkeley were asked to take 20 sniffs from a bottle containing androstadienone, which smells vaguely musky. Over a period of two hours, the volunteers provided five saliva samples from which cortisol levels were determined.

Compared to their response when sniffing a control odor (yeast), the women who sniffed androstadienone reported an improved mood and significantly higher sexual arousal, while their physiological response, including blood pressure, heart rate and breathing, also increased. This was consistent with previous studies.

In addition, however, the UC Berkeley researchers found that cortisol levels rose within about 15 minutes of sniffing androstadienone, and remained elevated for more than an hour.
We next need to look at other hormones that could explain the diversity of effects of androstadienone on sexual arousal and mood.

Just a few whiffs of a chemical found in male sweat is enough to raise levels of the stress hormone cortisol in heterosexual women, according to a new study by University of California, Berkeley, scientists.

The study, reported this week in The Journal of Neuroscience, provides the first direct evidence that humans, like rats, moths and butterflies, secrete a scent that affects the physiology of the opposite sex.

“This is the first time anyone has demonstrated that a change in women’s hormonal levels is induced by sniffing an identified compound of male sweat,” as opposed to applying a chemical to the upper lip, said study leader Claire Wyart, a post-doctoral fellow at UC Berkeley.

The team’s work was inspired by previous studies by Wyart’s colleague Noam Sobel, associate professor of psychology at UC Berkeley and director of the Berkeley Olfactory Research Program. He found that the chemical androstadienone – a compound found in male sweat and an additive in perfumes and colognes – changed mood, sexual arousal, physiological arousal and brain activation in women.


“Pheromones are chemical molecules expressed by a species aimed at other members of the species to induce stereotyped behavior or hormonal changes,” Wyart explained. “This male chemical signal, androstadienone, does cause hormonal as well as physiological and psychological changes in women. More cognitive studies need to be done to understand how androstadienone affects female cognitive functions.”

One implication of the finding is that there may be better ways to raise cortisol levels in patients with diseases such as Addison’s disease, which is characterized by low cortisol. Instead of giving the hormone in pill form, which has side effects such as ulcers and weight gain, “a potential therapeutic mechanism whereby merely smelling synthesized or purified human chemosignals may be used to modify endocrine balance,” the authors wrote.

Sweat has been the main focus of research on human pheromones, and in fact, male underarm sweat has been shown to improve women’s moods and affect their secretion of luteinizing hormone, which is normally involved in stimulating ovulation.

Other studies have shown that when female sweat is applied to the upper lip of other women, these women respond by shifting their menstrual cycles toward synchrony with the cycle of the woman from whom the sweat was obtained.

Androstadienone, a derivative of testosterone that is found in high concentration in male sweat, and in all other body secretions, has garnered the most attention. However, though its effect on a woman’s mood, physiological arousal and brain activity suggests that the chemical is a possible pheromone-like signal in humans, its effect on hormone levels was unknown.

Wyart and Sobel set out to test whether androstadienone affects hormone levels as well, focusing on the hormone cortisol. Cortisol is secreted by the body in times of stress, priming the body for “fight or flight.”

Wyart noted that, though this is the first time a specific component of male sweat has been shown to affect women’s hormones, other constituents of male sweat likely have a similar effect. The question remains: Which comes first – the change in cortisol level, which may induce a change in mood or arousal; or a mood change that increases cortisol levels?
“We next need to look at other hormones that could explain the diversity of effects of androstadienone on sexual arousal and mood,” she said.


Coauthors of the report include UC Berkeley undergraduates Sarah Wilson, Jonathan Chen and Andrew McClary; senior scientist Rehan Khan; and Dr. Wallace Webster, an otolaryngology resident at Kaiser Permanente Hospital in Oakland, Calif. The work was sponsored by the National Institute on Deafness and Other Communicable Disorders of the National Institutes of Health, and by the Army Office of Research.

How To Prevent Premature Ejaculation

Premature ejaculation
is one problem that can be devastating for a man’s self esteem and confidence.

The problem becomes compounded by the lack of self-esteem and eventually a man might begin to avoid sexual encounters altogether. It is necessary for a man to learn to control his state of arousal during intercourse to prolong his sexual encounter.

Many men believe that if they think about baseball or some other unconnected thought they would control their ejaculation. It is actually better if a man becomes more in tune with his body and learns to understand the signs and feelings that are associated with ejaculation. Once he learns to recognize the feelings he will be better able to control them and prolong his encounter.

Partners can work together to help the man recognize his points and stages of arousal and learn to back off. There are stages of sexual arousal that are felt at every part of the encounter. The trick to prolonging an encounter is to prolong the state of arousal that comes before the man reaches the point where he will not be able to control his ejaculation.

There are many methods that can be used to control ejaculation.

The man must train himself to recognize the signs in his body and learn to back off. To do this he might want to practice the method slowly by himself first. He can concentrate on how his body is feeling at each stage, which will help him to recognize it when he is with a partner. When he has mastered the ability to control his ejaculation while he is alone he can begin practicing with his partner. For this method to begin to work, it is necessary for the focus to be on the man. This will allow him to concentrate on his feelings without being overly concerned for his partner’s pleasure.

Once the feelings are understood and a greater understanding of the different phases of arousal and ejaculation are achieved, the couple will be able to move towards pleasing each other. An understanding partner is completely necessary for working on this problem. As you can see, putting aside her own pleasure to work on the problem that her partner is experiencing will lead to a more satisfying experience later.

Concentration on the entire body’s experience during sex is key to dealing with a premature ejaculation problem. It can be overcome with patience and understanding for the pleasure of both parties. Don’t become discouraged while you are working on the problem, it may take some time to work through.

You will both experience a greater pleasure at the end of the day and that will lead to a fuller experience. Perhaps, spending some time on pleasing each other once the problem is worked through is a great way to further enhance your sexual experience.

Amino acids and their significance for virility and libido


The intake of specific individual amino acids can stimulate and boost biological processes that are related to virility and sexual performance.1

Arginine and ornithine are the key amino acids in such cases. As precursors of nitric oxide, a molecule of which the far-reaching and remarkable effects on the body were only recently discovered (and for which a Nobel Prize was awarded in 19982), they have remarkable effects on the human body.

Increasing the arginine intake increases the nitric oxide level in the blood. This results, among other things, in a relaxation of the blood vessel walls and thus in improved blood circulation in general, and also in the penis.3

Although the stimulating effect of the amino acid arginine on sexual performance has been known for a while, it only came into the spotlight as a result of discussions on the well-known drugs to treat erectile dysfunction. Indeed, it has a similar effect, but is much cheaper and practically free of side effects.4

Arginine elevates the concentration of nitric oxide, which increases the arterial elasticity thus reducing the blood pressure and improving erectile processes.5

In contrast to the well-known chemically based pills to treat erectile dysfunction, arginine does not act immediately – the effect on virility becomes apparent after a few days; however, with the advantage that taking arginine is not associated with harmful side effects.4

The effects of arginine are highly diverse and beneficial:

 -  Improved erectile rigidity due to nitrogen retention.6
 -   Improved blood flow due to relaxation of the blood vessel walls.7
 -  Lowering of high blood pressure (and thus a worthwhile alternative to beta blockers and calcium antagonists that cause erectile dysfunction.8


Ornithine is degraded to arginine in the body. However, as it is degraded more slowly, its effects last longer. Ornithine is therefore the perfect complement to arginine.

Arginine and ornithine are usually synthesised in the bodies of healthy adults and are thus in adequate supply. However, if the production of these amino acids is disturbed by pathological changes or stress-related factors, the diet should be supplemented with foods containing arginine and ornithine.9

These amino acids are found in fish, meat, dairy products, nuts, rice, soybeans and wheat. Arginine and ornithine are also available as tablets, powders and even capsules. Particularly effective are combination products that contain other nutrients such as zinc, vitamin B and/or biotin.

Arginine – for an active love life!


Pharmacologists at the Hanover Medical School have studied the effect on patients with erectile dysfunction. “Initial evaluations indicate positive effects”, says head researcher Prof. Dirk Stichtenoth from the Institute for Clinical Pharmacology. Seventy volunteers received daily doses of arginine for four weeks and then placebos for another four weeks. The results of the study have been submitted to a medical journal for publication.

“At present there are no approved medicinal products for the treatment of erectile dysfunction containing arginine. This would require further studies. Arginine has only been available to date as a food supplement,” says the pharmacologist.10

The level was extremely low (9 pg/ml). The lab were actually uncertain whether the sample was male or female and so they quoted both the male and female reference ranges alongside the result. I guess Duane is not a common name in Germany and they couldn't distinguish whether it was male or female from the name (the result was so low that it could have been a female T level). However, our lower reporting limit to clients is < 46pg/ml (we find it causes too many complications and often can upset clients if we state how low some of them are). Levels this low would typically be found in a clearly hypogonadal man.”

With regard to the all-age reference range he is comfortably within the third quartile. This is a remarkable turnaround compared to his initial result. This is probably the most significant change we have seen in a client's T level, unless of course they have started pharmaceutical T replacement.

 How many male patients do you estimate that you have treated with testosterone therapy?

I would estimate that it is in excess of 200 over the years. It may or may not surprise you to know that I have also treated females who had shown via test procedures, a lack of testosterone.

What are your preferred delivery methods for prescription testosterone?

I very rarely resort to this method as over the longer term it shuts down the body’s own production of testosterone. When I have prescribed it – I have used a cream that is rubbed directly on the scrotum.

Doctors sometimes develop different areas of expertise. Do you feel that HRT (Hormone Replacement Therapy) is one of these areas for you and your office?

Yes, in both males & females.

What do you monitor on a routine basis when a man goes on HRT?

A full hormone profile is undertaken on a two to three month basis. It is always my intention to address the nutritional (in most cases) issues that cause hormonal imbalance. My intention is to use HRT as a short term measure while the body readjusts the mechanisms for its own hormone production.

 

How do you handle men who clearly could benefit from HRT but do not have insurance? What options are available through your office?

Erectile problems that are covered by insurance plans in the U.K. are extremely limited. However, the state will offer prescriptions for drugs such as Viagra.

 

How do you monitor prostate issues and screening for men on HRT?

I undertake an initial PSA screen & symptom analysis & I then always refer the patient to his doctor. I do like to follow that up with a consultation regarding the doctor’s results / advice.

How does your office go beyond simple subscribing of PDE5 Inhibitor for Erectile Dysfunction? Do you have any other treatment strategies for men who are experiencing a loss of erectile strength?

I always look at the nutritional issues that are, in most cases, at the root of the problem. I utilize my findings to address the issues together with natural proven supplementation for problems such as a lack of nitric oxide, low testosterone, etc. I always test for these levels prior to treatment.

Are there aspects of your practice or treatment strategies that you’d like to share that feel may make your practice "stand out from the crowd?"

That is a difficult question as so many healthcare practitioners have such differing strategies. Having said that, my aim is to restore the health an individual so that he may realize his genetic potential for great health & a long life. At a basic level the body is an immensely complicated manufacturing unit. In all my years of the study of it I am still in awe of what it has to accomplish to sustain life. The map of the metabolic pathways alone is absolutely mind boggling!

As an analogy, if you supply rubbish parts to manufacture say an automobile, you will get a rubbish vehicle that falls apart & dies prematurely. The body is no different – rubbish in equals rubbish out.

In the majority of cases erectile dysfunction is symptomatic of problems elsewhere, the latest research shows that healthcare professionals should always investigate cardiovascular disease if a patient presents with ED.

To reiterate, in my view it is vitally important to treat the ‘whole being’ rather that the symptoms of any particular issue.
